Lamisil is usually the first serious option your doctor brings up for toenail fungus. It is also the one people have the most questions about, because taking a daily pill for three months to fix a toenail feels like a big commitment. It is worth understanding what you are signing up for.
The short answer: Lamisil (the brand name for terbinafine) is an oral prescription antifungal and the standard medical route for toenail fungus. It works from the inside over a 12 week course. The tradeoffs are doctor supervision, possible liver monitoring blood work, and months of waiting while the new nail grows out. Here is what that means in practice, and where the at-home routine fits alongside it.
What Is Lamisil and How Does It Work?
It is terbinafine, a daily tablet taken for about 12 weeks for toenails. Rather than sitting on the nail surface, it travels through your bloodstream and builds up in the nail bed, reaching where creams cannot.
One thing nobody tells you clearly enough: the pill does not change the nail you can see. The damaged nail is already grown. The goal is that the NEW nail comes in healthy, and that takes far longer than most people are told. The 3mm-a-month figure quoted all over the internet is the fingernail rate. When researchers measured toenails directly, they grew about 1.62 mm a month. A great toenail is roughly 20 to 25mm from base to tip, so even a completely successful course means 12 to 18 months before the nail has fully replaced itself. The pill takes 12 weeks. The waiting takes over a year.
Does the Lamisil AT Cream Label Say Not to Use It on Nails?
Yes. Lamisil AT is the over the counter cream and it is a different product from the prescription tablet. Under Warnings, the DailyMed label for Lamisil AT cream reads "DO NOT USE on nails or scalp".
That trips a lot of people up. They see the Lamisil name on a pharmacy shelf, buy the cream for a toenail, and it is the wrong format for the job. The cream is labelled for athlete's foot, jock itch and ringworm, which are all skin conditions. A nail plate is not skin, and a cream sits on top of it.
So if a toenail is what you are dealing with, the shelf version is not the route. It is the tablet, a prescription topical made for nails, or a routine at home that gets under the nail edge rather than across the top of it.
What Are the Side Effects of Terbinafine?
Most commonly stomach upset, headache, rash, and a distorted or temporarily lost sense of taste. The reason it is supervised is the liver.
Terbinafine is processed by the liver, which is why many doctors order blood work before and sometimes during treatment. Serious liver problems are rare, but they are the reason this is a supervised prescription and not something you grab off a shelf.
This is exactly the tradeoff people weigh. We hear it from customers all the time, and you can find the same sentiment in any nail forum: some people are comfortable with a monitored course, and some decide their toenail is not worth a daily systemic medication. Both are legitimate choices to make with your doctor. Neither is wrong.
How Do You Know if Lamisil Is Working?
You look at the base of the nail, not the tip. New nail comes out of the matrix at the cuticle end, so the first sign is a band of clear, normal looking nail closest to the skin with the old damaged nail still ahead of it.
At 1.62 mm a month, that band is only a few millimetres by the time a 12 week course finishes. Take a photo every 30 days in the same light from the same angle. A month by month set shows the line moving even when a daily glance at your own foot shows nothing at all.
What does not tell you much is the tip of the nail. It is the oldest and most damaged part and it will still look wrong a year from now. If there is real doubt about whether the picture is changing, your doctor can send a nail clipping for testing.
Who Is Lamisil Usually Not For?
Anyone whose liver is already under strain, anyone on interacting medication, and anyone who simply does not want a systemic drug for a nail.
- People with liver disease or elevated liver enzymes
- People on medications that interact with terbinafine
- People who had a reaction to it before
- People who simply do not want a daily systemic drug for their toenails, which is a personal call your doctor will respect

Lamisil vs Jublia: What Is the Difference?
One is a pill, one is a brush-on. Lamisil works from the inside over 12 weeks with the strongest medical results and needs monitoring. Jublia is painted on daily for up to 48 weeks with no liver involvement, but it costs more and the numbers are more modest.
Worth knowing what "more modest" means precisely. Reading the CADTH clinical review for efinaconazole, Jublia's two phase 3 trials recorded mycological cure of 55.2% and 53.4% at week 52, but complete cure, meaning a clear nail and no detectable fungus at once, of only 17.8% and 15.2%. Where Does an At-Home Iodine Routine Fit?
Around the pill rather than instead of it, and especially through the long grow-out after the course finishes.
